- Part 1 of the form is for the employer or other addressee and contains identifying details of the notice such as the date of the notice , the name of the taxpayer, the address of the tax payer, the telephone number for the IRS and the reply address for the IRS
- Parts 2-5 are given to the Employee
- Parts 3 and 4 must be returned to the employer within three days
- Part 3 eventually goes to the IRS
- Part 4 stays with the employer
- Part 5 is the employee’s copy
- Part 6 is retained by the IRS
